Unfortunately, he couldn't speak English very well, therefore ECIRS was initially popularized only in Spanish-speaking countries. In 2003 I attended with professor Scarpa, my Director of the San Luigi University Hospital of Orbassano, a very practical course in Spain, organized by the other great father of endourology, Gabriel Valdivia Uria. There I met Gaspar, and immediately felt in love with ECIRS, because - after having seen a lot of prone PCNLs in my hospital - this appeared to me the easiest way to perform PCNL. Subsequently, we started to perform the first ECIRS in our hospital. Year after year we standardized all the steps, creating a method of this, and publishing many papers on ECIRS and on our technical modifications. The acme of our research was the publication of
the Atlas on ECIRS edited by Springer, along with my coworker and best help in our Clinic, Dr. Cecilia Maria Cracco, and with Andras Hoznek from France. We made this Atlas for discussing and reporting all the details of this combined approach, completely changing the PCNL perspective from radiological to urological, substituting the indirect fluoroscopic imaging with the direct endoscopic vision. I think that this is a relevant step forward, enabling us to perform safer surgeries. This is the short history of ECIRS, being Gaspar Ibarluzea for sure the father of the combined approach. With my team I had a very big role in popularizing ECIRS all over the world, and I am very, very happy that in 2024 almost 50% of the urologists are performing supine combined PCNL.
